INDIVIDUAL BEHAVIOR SUPPORT PLANS (IBSP)
Rapid and Responsive Initiation: All reports and plans are completed in a timely manner, whether they are required within 24 hours, 7 days, 30 days, quarterly, or at other specified intervals. In Community Care Homes (CCH) facilities, plans are initiated within 24 hours and completed within 7 days of admission. For Enhanced Behavioral Support Homes (EBSH) settings, plans are completed within 30 days, ensuring timely support tailored to the intensity of needs. This commitment to timely reporting guarantees consistent and responsive care for all consumers, adhering to all necessary deadlines and requirements.
Comprehensive Strategy Development: Each plan is grounded in Functional Behavioral Assessment, focusing on trauma-informed care and strategies that encourage functionally equivalent replacement behaviors. All plans are developed with active consumer participation and aim for the least restrictive methods.
